Donor I.D. Number
First Name
Last Name
Date of Birth (mm/dd/yyyy)
 
You can obtain this information from your donor ID card or pick up a form from the registration tech at your next donation

From I-71
Take 71 South to 471 South over bridge into Kentucky to Exit #2 Fort Thomas/Southgate. At bottom of ramp make a right hand turn. At the 1st stoplight make right hand turn. This is Fort Thomas Plaza. You will see a Jeff Wyler Dodge Dealership and we are in the back corner.

From I-75
If coming from 75 North or South, take I-275 (KY) East to 471 North. Take Exit #2 Fort Thomas/Southgate Exit. At bottom of ramp make a left hand turn. At 2nd stoplight make a right hand turn into Fort Thomas Plaza. You will see a Jeff Wyler Dodge Dealership and we are in the back corner.
 
From I-275
